Ready to start enrollment for your child?
Dear parents,
We are very happy that you choose Smartizen as your partner in your children's early education journey. Please complete student information from below to start with your enrollment. This is the first step in the registration process.
To complete the registration, please fill out the following forms:
- Student Application Form (required - this form as the first step)
- ACH Authorization Form for monthly fee payment (required - to fill and sign)
- Policy Guide (please review and sign)
- Waiver Form (please review and sign)
- Student Enrolment Agreement (please review and sign)
Once you finish form 1, forms 2-5 are sent to you with the email subject from info@smartizen.ca. Please check your spam or contact us if you can't find it.
