This is the first step in the registration process is to fill in the Student Information form.

To complete the enrolment process, please fill out the following forms:

1. Student Application Form (required - this form as the first step)
2. ACH Authorization Form for monthly fee payment (required - to fill and sign)
3. Policy Guide (please review and sign)
4. Waiver Form (please review and sign)
5. Student Enrolment Agreement (please review and sign)

Forms 2-5 are sent to you with the email subject "Welcome to Smartizen - Next Steps for Enrollment" from info@smartizen.ca. Please check your spam or contact us if you can't find it.
